化膿性リンパ節炎で発症した乳児自己免疫性好中球減少症の1例

A case of autoimmune neutropenia of infant induced of pyogenic lymphadenitis.

Ⅰ．はじめに

　自己免疫性好中球減少症の多くは乳幼児期に発症し，末しょう血中の抗好中球抗体により好中球の破壊が亢進し，好中球減少をきたす疾患である。頻度は10万人当たり1人で非常に稀な疾患とされている^1,2）。今回われわれは，頸部化膿性リンパ節炎を契機に診断された乳児自己免疫性好中球減少症の1例を経験したので，文献的考察を加えて報告する。

　We reported the case of a 8-month-old boy with autoimmune neuropenia induced by pyogenic lymphadenitis. Hematological examination showed a neutrophil count of 452 cells/μL and CT scan showed abscess formation in bilateral neck. We performed aspiration drainage two times but abscess was not completely disappeared. We additionally performed surgical drainage under general anesthesia and used granulocyte-colony stimulating factor（G-CSF）. Culture of the drainage fluid revealed Staphylocuccus aureus. He was discharged hospital on the 47th day.
