頸部膿瘍で発症した小児自己免疫性好中球減少症例

A case of autoimmune neutropenia of infant associated with neck abscess

Ⅰ．はじめに

　小児自己免疫性好中球減少症（autoimmune neutropenia of infant：ANI）は比較的稀な疾患で，その頻度は約1/10万人と報告されている^1～3）。またANIは，軽微な細菌感染を反復し，重症化するものは少ないと考えられている^1～3）。

　今回われわれは，頸部膿瘍で発症したANIの1例を経験したので報告する。

　The patient was an 1-year-old girl presented with a neck swelling. CT demonstrated an abscess of the right upper neck. At admission,white blood cell count was 8,500/μl,but neutrophils accounted for only 3.5％. Neutoropenia was thought to have caused the neck abscess. As anti-HNA-1a antibody was detected by the quantitative analysis of antineutrophil antibodies in her serum by flow cytometry,and the diagnosis of autoimmune neutropenia was made.
