上咽頭脂肪腫の1例

A case of huge nasopharngeal lipoma

Ⅰ　はじめに

　脂肪腫は全身あらゆる部位の脂肪組織から発生しうる良性腫瘍であるが，咽頭に発生することは少ない。特に上咽頭に発生することはきわめて稀であり，渉猟し得たかぎりわが国での報告例はこれまで1983年から2009年の間の5例のみである^1～5）。今回，われわれは上咽頭側壁に茎を有しその下端は喉頭蓋に達する巨大な上咽頭脂肪腫の1例を経験したので，若干の文献的考察を加えて報告する。

　Though lipoma is a common tumor in many anatomic sites throughout the body,its occurrence in the nasopharynx is extremely rare,with only five cases reported in Japan. We describe a 61-year-old man complaining of pharynx discomfort and snoring. Clinical examinations revealed that a large egg-shaped mass originating from the left lateral nasopharyngeal wall occupied almost entire nasopharyngeal and oropharyngeal space pushing forward the soft palate. The tumor was excised through transnasal endoscopic and transoral approaches. Removal of the pharyngeal part of the tumor improved his symptoms.
