持続的な白血球増多を伴った前頭洞癌の1例

A case of primary frontal sinus carcinoma with increase of white blood cell counts

Ⅰ　はじめに

　前頭洞癌は鼻・副鼻腔悪性腫瘍のなかでも発生頻度が低く，これまでの報告数も少ない稀な疾患である^1）。その解剖学的位置から早期診断は困難であるため進行した状態で発見されることが多く，また確立された治療法もないため予後はきわめて不良である^2）。一方，悪性腫瘍には顆粒球コロニー刺激因子granulocyte colony-stimulating factor（G-CSF）産生による白血球増多症を伴うことがあり，G-CSF産生腫瘍として報告されている^3）。G-CSF産生腫瘍は概ね予後不良であり，G-CSFが腫瘍増殖作用を有すると考えられている^3）。頭頸部領域でのG-CSF産生腫瘍の報告は少なく，G-CSF産生前頭洞癌の報告はわれわれが渉猟した限りこれまでにない。今回，われわれは持続的な白血球増多および血清G-CSF値の上昇を示し，放射線化学療法に抵抗性であり，8か月の経過で死亡した前頭洞癌の1例を経験したので，若干の文献的考察を加えて報告する。

　A 42-year-old male presented with a gradual swelling of the frontal region and pain. CT and MRI demonstrated a 45 mm tumor with intradural invasion. Histological diagnosis of biopsy specimen was squamous cell carcinoma. Although chemoradiotherapy with cisplatin and 5-fluorouracil,the patient died 8 months after the initial examination. Reported cases of frontal sinus carcinoma and granulocyte colony-stimulating factor（G-CSF）-producing carcinoma of the head and neck were reviewed,and the possibility of G-CSF-producing carcinoma was discussed.
