当科で経験した下口唇原発腺様囊胞癌の1症例

A case of adenoid cystic carcinoma of the lower lip

Ⅰ．はじめに

　腺様囊胞癌は頭頸部領域では唾液腺に好発する比較的稀な悪性腫瘍である。なかでも小唾液腺由来で口唇に発生することは特に少ないといわれている^1,2）。局所での発育は緩慢であるが，長期経過後に遠隔転移をきたす頻度が高く，一次治療後，局所再発する頻度も高いため，一般的に悪性度が高い腫瘍と考えられている。

　今回われわれは，下口唇に生じた腺様囊胞癌を手術および放射線療法の併用にて治療し，2年6か月間，無病生存をえている症例を経験したので若干の文献的考察を加えて報告する。

　Adenoid cystic carcinoma of the lip is relatively uncommon. A 61-year-old male complained of a mass of the lower lip. Histological diagnosis was adenoid cystic carcinoma. Extirpation of the tumor and reconstruction was performed. Abbe-Estlander method was adopted for reconstruction. After surgery radiotherapy was done because surgical margin was positive in the histological findings. He had with no recurrence at the time of this report. There is a few reports of adenoid cystic carcinoma of the lip.
