扁平上皮癌・甲状腺癌・血液疾患以外の頭頸部悪性腫瘍症例の検討

Ⅰ　はじめに

　頭頸部癌は，頭頸部のすべての臓器に発生するものを合計しても全癌の4～5％と比較的頻度の低い悪性疾患群である。頭頸部癌の組織型では扁平上皮癌が最も多いことはよく知られている^1～3）。また，甲状腺癌の頻度も比較的高く，扁平上皮癌と甲状腺癌に関しては多数の症例を集計した報告も多いため，診断と治療の方針は比較的確立していると考えられる。一方，頻度の低いその他の組織型の悪性腫瘍の疫学は，もともと頻度の低い頭頸部癌のうちさらに稀な疾患であるため，よく知られているとはいい難い。

　今回われわれは，当科のデータから扁平上皮癌や甲状腺癌に比べて圧倒的に症例数の少ない『その他の組織型』の頭頸部悪性腫瘍症例にはどのような組織型があるのか，そして，単一施設で症例を集計して結果を出せる可能性のある組織型と多施設による集計が望ましい組織型は何か，について検討したので報告する。

　Head and neck malignancies except for squamous cell carcinoma,thyroid carcinoma,and hematological disease treated in our department during 1991 and 2006 were clinically analyzed. Non-squamous cell,non-thyroid,non-hematological malignancies were 16.3％ of whole head and neck malignancies. The most common pathology was adenocarcinoma,followed by adenoid cystic carcinoma,and totally 34 pathologies existed in 151 patients. The most common site was the parotid gland in adenocarcinomas,and the submandibular gland in adenoid cystic carcinomas. Multi-institutional study should be considered for rare pathologies other than adenocarcinoma and adenoid cystic carcinoma.
