Maxillary swing approachにより摘出した上咽頭粘表皮癌の1例

Resection of nasopharyngeal mucoepidermoid carcinoma using the maxillary swing approach

Ⅰ．はじめに

　上咽頭癌では大多数の症例の組織型が放射線感受性の高い扁平上皮癌であるため，治療の主体は放射線療法で，近年は化学療法も併用される^1,2）。一方，腺系癌は上咽頭癌のうち0.6％程度で少ない^3）。組織型としては小唾液腺由来の癌が主で，一般に放射線感受性が低いと考えられるものの，手術が行われることは少なかった^4,5）。しかし，画像診断や手術手技の進歩によって，1980年代から切除術が優先されるようになってきた^5～7）。

　上咽頭癌手術のアプローチでは，経口蓋法，経上顎洞法，側方到達法の3つが知られている^4,7,8）。経上顎洞法の1つであるmaxillary swing approach（MSA）^9）は，上咽頭癌に対する低侵襲な外科療法として知られているが，わが国の報告はまだ少なく^10～12），主に放射線治療後の再発例や良性腫瘍に用いられている。

　今回，放射線感受性が低いと考えられる上咽頭粘表皮癌に対して，MSAで摘出した症例を経験したので報告する。

　We described our experience in a 56-year-old male with well-differentiated nasopharyngeal mucoepidermoid carcinoma using maxillary swing approach（MSA）. He underwent surgery followed by radiotherapy and had been free from recurrence for 3 years and 10 months after surgery. From our experience,MSA is one of the useful approaches to achieve minimum facial deformity and maxillary dysfunction. The therapeutic outcome should be assessed after long term follow up,since this type of nasopharyngeal carcinoma is rare and a low-grade malignancy.
