耳下腺MALTリンパ腫の1例

A case of MALT lymphoma in the parotid gland

Ⅰ．はじめに

　粘膜を介して外界と接する部位である消化管，唾液腺，気管支，胸腺などに形成されるリンパ組織は粘膜関連リンパ組織（mucosa-associated lymphoid tissue：MALT）と呼ばれる。このMALTを発生母地とする悪性リンパ腫がMALTリンパ腫である。MALTリンパ腫は多くは消化管，なかでも特に胃に発生するが，頭頸部領域では唾液腺，眼付属器，甲状腺などに発生する^1）。今回，われわれは比較的稀な耳下腺に発生した高悪性度のMALTリンパ腫の1症例を経験したので，若干の文献的考察を加えて報告する。

　Lymphomas arising in mucosa-associated lymphoid tissue（MALT）are reffered to as MALT lymphomas. The most frequent site of MALT lymphomas in gastrointestinal tract is the stomach, but they are also known to occur in the salivary glands and thyroid glands in the head and neck area. A 77-year-old female presented with a palpable 20×20mm mass in her left parotid gland. A left superficial parotidectomy was performed. The pathological findings were consistent with MALT lymphoma. After surgery, chemotherapy and radiotherapy was administered for high grade MALT lymphoma. The recurrence was not observed for 13 months postoperatively.
