頭蓋外内頸動脈瘤の1例

A case of extracranial internal carotid artery aneurysm

Ⅰ．はじめに

　内頸動脈瘤は頻度の高い疾患であるが，頭蓋外の内頸動脈に発生する動脈瘤は非常に稀である^1,2）。頭蓋外内頸動脈瘤は頸部の拍動する無痛性腫瘤として発見されることが多いが，塞栓や血栓による脳梗塞，一過性脳虚血などの中枢神経症状を引き起こす可能性もあり，慎重な対処が必要である^2～4）。また，下位脳神経や交感神経を腫瘤が圧迫することで嚥下障害や嗄声とHorner徴候などの症状を呈することもある^3）。発生要因としては，外傷，動脈硬化，細菌感染，梅毒，線維筋性異形成，Marfan症候群，大動脈炎症候群，手術操作，放射線照射などが挙げられる^5）。近年では外傷性および動脈硬化性のものが大部分を占める^1,2）。今回，われわれは扁桃周囲膿瘍を疑わせる局所所見を呈した中咽頭へ突出する頭蓋外内頸動脈瘤を経験したので報告する。

　Extracranial carotid artery aneurysms are uncommon. An 83-year-old man presented with sore throat. In other hospital, he was misdiagnosed with peritonsillar abscess, and recived needle aspiration. A giant aneurysm was recognized near the right wall of oropharynx by CT scan, MRI and angiography. Because of his age, complication and the size of aneurysm, he was followed closely without treatment.
