MRSAおよび緑膿菌感染を伴った好酸球性中耳炎の1例

A Case of Eosinophilic Otitis Media with MRSA and P.aeruginosa Infection

I．はじめに

　近年，食生活の欧米化や気密性の高い住居での生活が増加するに伴い，アレルギー性疾患の患者は増加し，アレルギーが基礎にある難治疾患も注目されるようになってきている。好酸球性中耳炎はその1つであり，喘息などを基礎疾患として生じる難治性中耳炎である。未だ原因も十分に解明されておらず，長期的に有効な治療法もないのが現状である。

　われわれは好酸球性中耳炎にMRSA（meticillin resistant staphylococcus aureus）および緑膿菌感染を伴った症例を経験した。好酸球性中耳炎，緑膿菌，MRSA中耳炎はいずれも治療に難渋する中耳炎であり，これらが合併する例は少ない。文献的考察を含め治療経験を紹介する。

　A patient has suffered from asthma for 16 years,ｃｈｒｏｎｉｃ otitis media and chronic sinusitis. He has been treated with Macrolides for several years. After sinus surgery,he had bilateral aural fullness and discharge. MRSA and P.aeruginosa were detected in culture from aural and nasal discharge. The patient was treated with several antibiotics,however,his chronic otitis media was not cured. To remove tympanic mucosa totally and prevent otitis media from infection,his left ear was operated. Tympanoplasty improved his aural fullness and discharge.
