アレルギー性鼻炎に対する手術療法―超音波凝固切開装置による治療経験

Surgical Treatment for Allergic Rhinitis:Application of the Harmonic Scalpel

I．はじめに

　近年，アレルギー性鼻炎に対するレーザーを用いた日帰り手術は急速な勢いで普及し，通年性のハウスダスト（HD）アレルギー^1～4）のみでなくスギ花粉症^5）に対しても有効な成績が報告されてきている。しかし，われわれの過去の検討^1,2）では，無効例が全体の約30％存在していた。

　最近われわれは，このような難治性のHDアレルギー性鼻炎症例に，山西ら^6）や柳ら^7）の報告を参考にして，超音波凝固切開装置（ハーモニックスカルペル^TM：以下HSと略）を用いて下甲介粘膜の凝固手術を日帰りで行っている。ここでは，この手術の概要と治療成績の一部を呈示する。

　The harmonic scalpel（an ultrasonic scalpel） has been developed as a surgical device to cut and coagulate tissue by utilizing ultrasound. We coagulated the inferior turbinate mucosa using the harmonic scalpel in 12 patients with intractable allergic rhinitis. This method was safe,and the excellent results were obtained as an outpatient procedure. In future,if the blade will be made smaller,this type of surgery may be used widely for allergic rhinitis.
