巨大な上皮小体癌の1症例―その診断および反回神経の走行

A Case of Giant Carcinoma of Parathyroid

I．はじめに

　上皮小体癌は比較的稀な疾患であり，原発性上皮小体機能亢進症の中で癌の占める割合は0.6～4％程度といわれている^1）。上皮小体癌は一般に腺腫や過形成と比較して大きいものが多いが，50mmを超えるものは10％程度と報告されている^2）。

　今回われわれは，直径60mmの巨大な上皮小体癌を経験した。その症例を報告するとともに，上皮小体癌の診断および反回神経の走行について考察した。

　A 41-year-old woman with giant carcinoma of the left parathyroid was complaining of a swelling of her left neck. CT and MRI showed a tumor pressing the left thyroid.

　Surgical treatment was performed without injury of the recurrent laryngeal nerve. It was difficult to make preoperative judgement with certainty whether the recurrent laryngeal nerve was located above or below the parathroid tumor.

　Therefore,we should take care on the position of recurrent laryngeal nerve when we perform an operation for parathyroid tumor.
