鼻・副鼻腔内向型乳頭腫に対する内視鏡下鼻内手術

Endoscopic Sinus Surgery for Inverted Papillomas

　はじめに

　鼻・副鼻腔内向型乳頭腫（inverted papilloma）は良性腫瘍であるが，再発しやすく，組織破壊性があり，癌の合併や癌化の可能性もあることから，臨床的には悪性腫瘍に準じた治療が施行されることが多い。一方，近年内視鏡下鼻内手術（以下，ESSと略）の普及と進歩に伴い，内向型乳頭腫に対するESSの適応について検討されるようになってきた^1〜6）。

　今回われわれは，内向型乳頭腫3例に対しESSを施行し，本疾患に対するESSの適応と限界および手術に際しての注意点などについて検討したので，若干の文献的考察を加え報告する。

Medial maxillectomy via lateral rhinotomy approach has been the standard treatment for inver-ted papilloma of the nose and paranasal sinuses.The authors treated three patients with inverted papilloma with endoscopic sinus surgery. All patients were found no carcinoma. Two patients with recurrent tumor in the ethmoid sinus under-went additional endoscopic sinus surgery. Based on our experience, the limited lesions in the nasal cavity, the ethmoid and sphenoid sinuses (including recurrent case) have good indication of endoscopic sinus surgery.
