 We reported a case of one-year and two-month old boy with severe acute otitis media caused by Penicillin resistant Streptococcus pneumoniae. Immunological studies revealed panhypogammaglobulinemia and nearly total absence of circulating B cells. Deficient expression of the Bruton's thyrosine kinase(BTK) protein in monocytes and mutation in the BTK gene was identified,leading to the diagnosis of X-linked agammaglobulinemia. After the diagnosis,intravenous gammaglobulin replacement therapy has been started. Although the congenital imminodeficiecy diseases are not common,careful attention should be paid to whether the individual patient with upper respiratory infections including otitis media might have the diseases.
