ペニシリン耐性肺炎球菌による急性中耳炎で発見されたX連鎖無ガンマグロブリン血症の1例

A Case of X-Linked Agammaglobulinemia Detected with Acute Otitis Media Caused by Penicillin Resistant Streptococcus pneumoniae

I．はじめに

　X連鎖無ガンマグロブリン血症（X-linked agammaglobulinemia：XLA）は，X染色体上のBTK遺伝子変異を病因とする抗体産生不全症である^1～3）。BTK遺伝子産物であるBrutonのチロシンキナーゼ（BTK）はB細胞分化に必須であり，この欠損により末梢血B細胞は著減し，ガンマグロブリンが著明に低下する^1～3）。一方，急性中耳炎は，3歳までに50～71％の児が少なくとも1回は罹患するといわれる一般的な疾患である^4）。しかし近年，ペニシリン耐性肺炎球菌（penicillin resistant Streptococcus pneumoniae：PRSP），βラクタマーゼ非産生アンピシリン耐性インフルエンザ菌（β-lactamase-negative ampicillin resistant Haemo-philus influenzae：BLNAR）などの耐性菌により，遷延，重症化し，反復する乳幼児例が珍しくなくなってきた^4～6）。

　今回，われわれはPRSPによる重症急性中耳炎を契機として発見されたXLAの1例を経験したので報告する。

　We reported a case of one-year and two-month old boy with severe acute otitis media caused by Penicillin resistant Streptococcus pneumoniae. Immunological studies revealed panhypogammaglobulinemia and nearly total absence of circulating B cells. Deficient expression of the Bruton's thyrosine kinase（BTK） protein in monocytes and mutation in the BTK gene was identified,leading to the diagnosis of X-linked agammaglobulinemia. After the diagnosis,intravenous gammaglobulin replacement therapy has been started. Although the congenital imminodeficiecy diseases are not common,careful attention should be paid to whether the individual patient with upper respiratory infections including otitis media might have the diseases.
