術前に診断し得た囊胞状リンパ節転移を伴った甲状腺乳頭癌の1例

Cystic Nodal Metastasis from Thyroid Papillary Carcinoma―A Case Report

I．はじめに

　側頸部の囊胞性病変の鑑別診断としては，側頸囊胞，結核性リンパ節炎，囊胞性リンパ管腫，悪性リンパ腫などが挙げられるが，甲状腺乳頭癌のリンパ節転移が囊胞変性することも稀ではあるが存在することが知られている^1～5）。

　今回われわれは，両側頸部の囊胞性病変を契機に発見された甲状腺乳頭癌の1例を経験したので報告する。

　A 30-year-old man was complaining of a swelling of bilateral lateral neck. CT scan showed cystic tumors in bilateral neck and US showed thyroid tumor. Thyroid tumor was diagnosed as papillary carcinoma by fine needle aspiration cytology. Thyroglobulin level in content of bilateral cervical cyst was high.

　Bilateral cervical cyst as cervical lymph node metastases from thyroid papillary carcinoma was diagnosed. Total thyroidectomy with bilateral neck dissection was performed.
