齲歯が原因で巨舌症を呈したと考えられた小児舌リンパ管腫の1例

Lymphangiomatous macroglossia caused by decayed teeth

I．はじめに

　リンパ管腫はリンパ管の増殖からなる腫瘍性新生物であり，大部分は先天的な組織異常（過誤腫）として生ずると考えられており，乳幼児期に発生することが多い^1,2）。舌リンパ管腫のなかでびまん性のものはしばしば巨舌症を呈し，咀嚼障害や構音障害など口腔機能へ障害を及ぼすことがあるため，その治療を行う際には慎重な対応が望まれる^2）。今回われわれは齲歯が原因で巨舌症を呈したと考えられる小児舌リンパ管腫の症例を経験したので報告する。

　Lymphangiomas are uncommon developmental anomalies and involvement of the tongue is rare. But lymphangioma of the tongue is the most common cause of macroglossia. We present a case with lymphangiomatous macroglossia causing difficulty swallowing,drooling and slurred speech. She had many decayed teeth and we speculated that these bad teeth could cause macroglossia. Treatment of decayed teeth significantly improved macroglossia and now she has no complaints. Less aggressive,symptomatic therapy may be an alternative to avoid multiple surgery.
