突発性難聴に対するPGE1の有効性―ステロイド無効例への使用―

Effect of PGE1in the Treatment of Sudden Deafness:―A Clinical Study of PGE1for Cases in which Steroid was Ineffective―

I．はじめに

　突発性難聴に対して，これまでに確立された治療法はなく，ステロイド，循環改善剤，高圧酸素療法，星状神経節ブロック，ビタミン剤など種々の治療が各施設ごとに行われているのが現状である。この中でも，ステロイドが治療の主体となっている施設は多い。しかし，症例によってはステロイドで十分な効果が得られないことがある。PGE₁は内耳の血流を改善する目的で突発性難聴の治療に使用されてきた^1～3）。これまでにも，ステロイド無効例に対してPGE₁を投与したという報告はあるが^4,5），これらはステロイドを7日間以上投与したのちに，PGE₁の投与を開始している。

　今回，ステロイドによる治療開始3日目と，より早期の段階でステロイドの効果判定を行い，無効例に対してPGE₁の併用投与を開始した。その結果，良好な聴力成績を得ることができた。突発性難聴に対する新しい治療法の1つとして提案したい。

　We studied the effect of PGE₁ for those cases in which hearing level had not been improved by steroid. Between April 2001 and May 2003,we treated 19 cases with sudden deafness at our hospital. The delayed administration of PGE₁ could be an effective therapy for the cases in which steroids were ineffective. We also emphasized that we should check the hearing level at the third day of the therapy and that we should administer PGE₁ to the cases not responded to steroids
