呼吸困難を生じた巨大な咽後血腫の1例

A Gigantic Retropharyngeal Hematoma ProducingDyspnea

I．はじめに

　咽後血腫は，非炎症性の急速な頸部腫脹としては非常に稀な疾患である^1,2）。しかし，その病変は拡がりやすく，時に上気道の閉塞をきたすため，緊急で挿管，気管切開などの気道確保が必要となる。

　今回われわれは，外傷などの明らかな原因がなく咽後血腫を生じ，呼吸困難により気道確保が必要になった症例を経験したので，文献的考察を加え報告する。

　A 40-year-old patient presented with a retropharyngeal hematoma without any contributing factors,such as trauma. Dyspnea required securing the airway.

　A long-term hemodialysis-related platelet dysfunction and an anticoagulant used during dialysis might have been involved in the pathogenesis.
