ウイルス性髄膜炎（肥厚性硬膜炎）による混合性喉頭麻痺の1例

Laryngeal Paralysis Associated with Viral Meningitis:A Case Report

　はじめに

　従来，肥厚性硬膜炎は，硬膜の慢性炎症性肥厚を呈する稀な疾患であるといわれていたが，近年MRIの普及によりその報告は増加している^1〜6）。

　今回われわれは，急性に混合性喉頭麻痺を呈した症例で，髄液単核細胞数の増加とMRIでの硬膜の肥厚所見などからウイルス性髄膜炎（肥厚性硬膜炎）と診断し，プレドニゾロン投与で麻痺の改善した症例を経験したので報告する。

A 62-year-old man was complaining of dysphagia andhoarseness. Disturbance of elevation of the right softpalate and right recurrent nerve paralysis (dysfunction ofthe right IXth and Xth cranial nerves) were noted.Noother meningeal signs or neurological abnormalitieswere detected.

Monocyte counts increased in cerebrospinal fluid.MRI showed thickening of the dura mater.Serologicalexamination of paired blood samples revealed a signifi-cant increase in the antibody titer against varicella-zos-ter virus.A diagnosis of vira lmeningitis (hypertrophicpachymeningitis) was made.Nerve paralysis improvedafter the administration of prednisolone.
