口蓋垂を温存した口蓋垂軟口蓋咽頭形成術

UPPP without Uvulotomy for Avoiding Postoperative Nasopharyngeal Regurgitation

Ⅰ．はじめに

　Obstructive sleep apnea syndrome（OSAS）に対する治療として，閉塞部位が中咽頭にある患者に対しては，口蓋垂軟口蓋咽頭形成術（UPPP）が一般的に行われ，口蓋垂の切除あるいは半切が行われている^1）。他施設で，UPPPあるいはレーザーによる口蓋垂軟口蓋形成術を受けた患者で「無呼吸，いびきは軽くなり，ゆっくり飲み込めるが，とっさに飲み込むと鼻から出てきて困る」と訴える患者が多く，当科を受診した症例では22例中17例で不具合がみられた。このような術後の鼻咽腔閉鎖不全を回避することを目的として，口蓋垂全体を温存するUPPPを考案した。

UPPP includes uvulotomy. Complications after removal of the uvula were stenosis of the pharynx,regurgitation of food into the nose. I developed the procedure“UPPP without uvulotomy”By this operation,the complications,regurgitation of food into the nose could be avoided. As for the changes indicated by sleep monitoring,minimum SpO₂ was increased significantly,and snore was decreased.
