経鼻胃管チューブ挿入時に発症した頸部皮下気腫・縦隔気腫例

A case of subcutaneus and mediastinal emphysema following insertion of a nasogastric tube

I．はじめに

　頭頸部癌の周術期や，放射線治療中の粘膜炎による経口摂取困難時に経鼻胃管チューブによる代替栄養は日常的に行われる方法である。今回われわれは，経鼻胃管チューブ挿入時に頸部皮下気腫と縦隔気腫をきたした症例を経験したので報告する。

A 84-year-old woman developed a swelling and pain of her neck after insertion of nasogastric tube. CT-scan revealed subcutaneus and mediastinal emphysema in the parapharyngeal and retropharyngeal space. The cause was the nasogastric tube perforating the mucosa of nasopharynx,oropharynx or hypopharynx and air was trapped in the parapharyngeal and retropharyngeal space. On the seventh day ,emphysema disappeared on CT.
