大分県におけるハナヒルDinobdella feroxのヒト結膜嚢寄生例

Infestation by a young leech, Dinobdella ferox, in the conjunctival sac in a child in Oita Prefecture

　10歳男児が，左眼結膜の異物に気付き受診した。細隙灯顕微鏡検査で，左眼上方の結膜円蓋部に体長約10mmのヒル様の吸盤をもった動き回る虫体があり，その周囲の球結膜に軽度の充血があった。外眼部その他にこれ以外の異常はなかった。摘出した虫体を検索した結果，幼生期のハナヒルDinob—della feroxと同定した。受診の約5日前に患者は川で泳いでおり，その際に結膜嚢にハナヒルが侵入して寄生したと推定された。大分県では最初のハナヒルによるヒト寄生例である。

A 10-year-old boy presented with a foreign body in his left conjunctiva. Slit-lamp microscopy showed a moving worm resembling a leech in the upper fornix. It had a sucker and was 10 mm in length. His eye was free of dis-orders except slight hyperemia at the site of the worm. The worm was identified as a young leech of Dinobdella ferox. The infestation probably occurred while swimming in a stream 5 days before. This is the first reported case in Oita Prefecture.
