　49歳の男性がパワーショベルの修理中，グリースが高圧で噴射して右眼に当たった、，翌日の受診時，右眼は視力O.06，眼圧は35mmHgであった。右眼に眼球突出と高度の眼瞼腫脹があり，眼球不動であった。虹彩離断，虹彩炎，散瞳があった。CT検査で異物は同定されなかった。眼球突出と眼瞼腫脹は持続した。6週後のCT検査で，下眼瞼から眼窩内に，グリースによると推定される肉芽腫が発見された。観血的にグリースを吸引除去したのち，眼球突出と眼瞼腫脹は軽減し，眼球運動もほぼ正常化した。

A 49-year-old male suffered injury to his right eye by a jet of grease while trying to repair a power-operatedshovel. When seen the following day, his visual acuity was 0.06 right with the intraocular pressure of 35mmHg. Theaffected eye showed proptosis, eyelid swelling, iridodialysis, iritis and mydriasis. The ocular motility was absent. Computedtomography (CT) failed to identify a foreign body. After 6 weeks of persistent proptosis and eyelid swelling, CT showeda granulomatous mass from the lower eyelid to the orbit. The granuloma appeared to be an encapsulated grease material.Drainage of the grease resulted in reduction of proptosis and eyelid swelling and in normalized ocular motility.
