緑内障眼における近視型乳頭と非近視型乳頭間の傍乳頭網脈絡膜萎縮の比較解析

Differences in peripapillary atrophy between eyes with and without myopia-shaped disc in normal-tension glaucoma

　耳側コーヌスなど近視型乳頭が，緑内障性傍乳頭網脈絡膜萎縮に及ぼす影響を検索した。対象として，正常眼圧緑内障患者で近視型乳頭のある24例24眼と，これがない21例21眼を選んだ。傍乳頭網脈絡膜萎縮を網膜断層計を使って画像解析し，視野障害との関連を比較した。近視型乳頭では非近視型乳頭に比べ，同程度の視野障害があっても傍乳頭網脈絡膜萎縮が大きいが，視野障害指数との相関は小さかった。正常眼圧緑内障での傍乳頭網脈絡膜萎縮の病態生理学的意義が，近視型乳頭の有無によって異なる可能性がある。

We evaluated the role of myopia-shaped disc in glacomatous peripapillary atrophy. Out of patients of nor-mal-tension glaucoma, we selected 24 eyes with myopia-shaped disc and 21 eyes without. The optic disc was sub-jected to image analysis including tomography of the retina. The findings were compared with visual field defects. Eyes with myopia-shaped disc showed greater peripapillary atrophy than those without showing a similar visual field defect. Parameters for peripapillary atrophy were not significantly correlated with visual field indices in eyes with myopia-shaped disc. There is a possibility that glacomatous peripapillary atrophy may have different pathophysiological signifi-cances between eyes with or without myopia-shaped disc.
