プリズムレンズ装用による半盲治療

Prismatic treatment for hemianopia

　後大脳動脈領域の脳梗塞により同名半盲となった男性6例，女性2例の計8例と視神経疾患により水平半盲となった女性2例に対しプリズムレンズの基底を盲側として装用させた。プリズムレンズ装用により，盲側の視野情報を健常側網膜に投影させ，同名半盲および水平半盲の視野が拡大するか否かを検討した。ゴールドマン視野計を用いた視野の測定では，プリズムレンズ装用により全例で視野が盲側にずれ，中心部の視野拡大が認められた。さらに，10プリズムジオプタのFresnelの膜プリズムを両側眼鏡に装着することにより，日常生活に十分な視野の拡大を認めた。Fresnelの膜プリズムは安価で軽く，着脱も簡易であることから，半盲の矯正法として実用的な方法と考えた。

We prescribed plastic paste-on Fresnel prisms to 8 patients with homonymous hemianopia and 2 patients with horizontal hemianopia. The base of 10-diopter prism lay towards the direction of visual field defect. This procedure induced shift of peripheral visual field towards the central meridian or the horizontal line. It also resulted in significant improvements in activities of daily living. Plastic Fresnel prism is inexpensive, easily applicable, and promises to be a useful aid for binocular hemianopia.
