要約　目的：ソフトコンタクトレンズ（SCL）装用中に発症したアカントアメーバ角膜炎3症例の報告。症例と経過：3症例とも女性で，年齢は32，33，44歳であった。それぞれ毎日装用使い捨てSCL，連続装用使い捨てSCL，連続装用非含水性SCLを使用していた。連続装用をしていた2例は，SCLを水道水で洗浄していた。いずれも片眼に発症し，2例では放射状角膜神経炎，1例には偽樹枝状病変があった。2例では角膜擦過標本にアカントアメーバが検出された。全例に抗真菌薬の局所投与と角膜搔爬を行い，2例にはクロルヘキシジンを点眼した。抗真菌薬と角膜搔爬が奏効しない1例にはpolyhexamethylene biguanide（PHMB）点眼を追加し，急速な消炎を得た。結論：SCL長期装用に伴うアカントアメーバ角膜炎では早期診断と早期治療が望ましい。抗真菌薬と角膜搔爬が無効な難治例にはPHMB点眼が奏効することがある。

Abstract.　Purpose：To report 3 cases of acanthamoeba keratitis secondary to prolonged use of soft contact lens. Cases and Findings：All the cases were females aged 32, 33 and 44 years respectively. All were unilaterally affected by keratitis. They had been wearing soft contact lens for several years. Radial keratoneuritis was present in 2 cases and pseudodendritic lesion in one. Acanthamoeba was detected in the smear of corneal scrapings in 2 cases. All the 3 cases were treated by local antifungal agent and scraping of cornea. Two cases received instillation of chlorhexidine. One case, who failed to respond to these treatments, showed rapid recovery after additional instillation of polyhexamethylene biguanide（PHMB）. Conclusion：Early diagnosis and treatment are essential for acanthamoeba keratitis in soft contact lens wearers. Topical PHMB may be effective for intractable cases.
