他の眼圧下降薬で眼圧コントロールが得られない緑内障150名150眼にラタノプロスト点眼を行った。内訳は,原発開放隅角緑内障83眼,正常眼圧緑内障41眼,続発緑内障12眼,その他14眼であり,緑内障手術の既往が12眼にあった。点眼開始から2か月までのoutflow pressureの改善が30%以上を著効,10%以上を有効,10%未満を不変とした。有効群と著効群のうち,それ以後に眼圧が点眼開始前の値以上になった場合を効果減弱とした。有効は119眼(79%)で,うち著効が63眼(42%)であり,不変が27眼(18%),点眼中止が4眼であった。効果減弱は有効群の56眼(47%),著効群の18眼(29%)に認められた。著効群での効果減弱に影響する因子は,併用薬の数と点眼開始前の眼圧であった。
We evaluated the effect of latanoprost for glaucoma with insufficient control of intraocular pressure (IOP) in 150 eyes of 150 patients. The series comprised primary open-angle glaucoma 83 eyes, normal-tension glaucoma 41 eyes, secondary glaucoma 12 eyes and others. The effect was defined as very effective when the outflow pressure improved by 30% or more, as effective when it improved by 10% or more, and as ineffective when it was less than 10%. The effect was further defined as reduced when the IOP rose to or above the pretreatment level after initial improvement. Within 2 months of latanoprost instillation, 119 eyes (79%) were judged as effective including very effective 63 eyes (42%), leaving ineffective 27 eyes (18%) and 4 drop-out eyes. Reduced effect was observed in initially effective 56 eyes (47%) including very effective 18 eyes (29%). Reduced effect in very effective eyes was mainly due to the number of concomitantly used medications and the pretreatment IOP level.
