増殖糖尿病網膜症に伴う傍中心窩毛細血管網閉塞に対する硝子体手術の効果

Outcome of vitrectomy for occluded perifoveal capillary network in diabetic retinopathy

　傍中心窩毛細血管網閉塞のある虚血性糖尿病黄斑症22眼に硝子体手術を行った。術後，蛍光眼底造影で評価した中心窩毛細血管網閉塞は，6眼で再疎通し，14眼では不変，2眼では拡大した。再疎通6眼での視力は3眼で2段階以上改善，3眼で不変であり，悪化はなかった。全22眼での視力は，7眼で改善，14眼で不変であり，1眼で悪化した。虚血性糖尿病黄斑症に対して硝子体手術が有効である可能性を示す症群である。

We performed pars plana vitrectomy on 22 eyes of ischemic diabetic maculopathy with occluded perifoveal capillary network. When evaluated by fluorescein angiography, the perifoveal capillary occlusion became reperfused in 6 eyes, remained unchanged in 14 eyes, and enlarged in 2 eyes. In 6 eyes with reperfusion, the visual acuity improved by 2 lines or more in 3 eyes and remained unchanged in the other 3 eyes. In 22 eyes in the series, the visual acuity improved in 7 eyes, remained unchanged in 14 eyes, and deteriorated in one eye. These cases illustrate that vitrectomy may be effective for ischemic diabetic maculopathy.
