傍中心窩網膜毛細血管網閉塞を伴う網膜静脈分枝閉塞症に対する硝子体手術後の血管再疎通と視力経過

Course of visual acuity and recanalization of occluded perifoveal capillaries following vitrecomy for branch retinal vein occlusion

　傍中心窩毛細血管網閉塞のある網膜静脈分枝閉塞症38眼に硝子体手術と後部硝子体剥離術を行い，視力経過と虚血改善効果を検討した。最終視力は，2段階以上の改善19眼，不変17眼，悪化が2眼であった。最高視力に達するまでの期間は，6か月未満が17眼6か月以上12か月未満が8眼，1年以上が13眼であった。中心窩周囲の毛細血管閉塞は，再疎通21眼，不変15眼であり，悪化はなかった。網膜静脈分枝閉塞症に対する硝子体手術により，視力と傍中心窩毛細血管網閉塞が改善することがあると結論される。最高視力に達するまでに1年以上が必要な事例がある。

We performed vitreous surgery in 38 eyes of branch retinal vein occlusion. All the eyes had occluded peri-foveal capillaries. The final visual acuity improved in 19 eyes, remained unchanged in 17 eyes, and deteriorated in 2 eyes. The maximum visual acuity was obtained within 6 months of surgery in 17 eyes, between 6 and 12 months in 8 eyes, and after one year in 13 eyes. The occluded perifoveal capillaries became recanalized in 21 eyes and remained un-changed in 15 eyes. The findings show that vitrectomy may induce improvement in visual acuity and recanalization of perifoveal capillaries in branch retinal vein occlusion. It may take one year or longer before attaining maximum visual acuity.
