A 57-year-old male was admitted to the neurosurgery ward due to dizziness and impaired gait of 10 days' duration. Magnetic resonance imaging (MRI) showed multiple intracerebral lesions. He was referred to us for myodesopsia in the left eye 6 days later. His visual acuity was 1.2 in either eye. The left eye showed vitreous floaters and an exudate with hemorrhage temporal to the disc. The exudate enlarged from 0.5 disc diameter (DD) to 1 DD 2 weeks later. Brain surgery showed swollen astrocytes but no signs of tumor or inflammation. Systemic antibiotics induced improvement in neurological signs, intracranial lesions and fundus exudate. Since no definite diagnosis could be made regarding the cerebral and ocular lesions. Simultaneous improvement in both lesions seemed to suggest some underlying systemic disorder.
