網脈絡膜病変を伴ったWegener肉芽腫症

Wegener's granulomatosis associated with retinochoroidal lesions

　41歳の男性で，網脈絡膜病変を伴ったWegener肉芽腫症（limited form）を経験した。初診時，両眼の後極部に浮腫状混濁が散在していた。CT検査で鼻腔から上顎洞にかけて異常陰影があり，胸部X線検査では両肺野に多数の結節状陰影があった。腎病変はなかった。ステロイド剤の全身投与は効果がみられなかった。シクロスポリンの内服治療により上気道病変，肺病変および両眼底の浮腫状混濁は次第に軽快した。本症例の両眼底にみられた浮腫状混濁は，Wegener肉芽腫症による血管炎とそれに伴う肉芽腫様変化が，脈絡膜にも起きたためのものではないかと思われた。

A 41-year-old male presented with blurring of vision in both eyes. Ophthalmoscopy showed multi-ple edematous lesions in the posterior fundus in both eyes. Computed tomography showed shadows in the nasal cavity and maxillary sinus bilaterally. Roentgenogram showed numerous nodular opac-ities in both lungs. He was diagnosed as limited form of Wegener's granulomatosis. Systemic cor-ticosteroid was futile. Treatment with cyclosporin was followed by gradual healing of pulmonary and macular lesions. Presumed choroidal vasculitis with granulomatous affections seemed to have induced the observed retinochoroidal lesions.
