We reviewed 126 eyes of pseudophakic retinal detachment. In eyes with intraoperatively ruptured posterior capsule, development of retinal detachment within 3 months after surgery was significantly more frequent after phacoemulsification than extracapsular extraction. Intraoperative troubles including ruptured posterior capsule or out-of-the-bag fixation were significant factors in retinal detachment within 4 to 6 months after surgery and in cases with retinal tear due to granular tissue or focal pigment. Vitreoretinal traction was the cause in these eyes. Conventional types of retinal detachment were more common after extracapsular cataract extraction. The findings show the importance of prophylactic treatment of retinal lesions predisposing to breaks and of careful cataract surgery to avoid vitreous loss to prevent pseudophakic retinal detachment.
