外眼部所見に乏しい，網膜中心動静脈断裂と考えられた眼球打撲の1例

A case of traumatic occlusion of central retinal artery and vein without involvement of external eye segment

（展示239）　2歳女児が，自宅の机の角で左眼を打撲した。眼瞼の擦過症と皮下出血があったが軽微であった。2日後に当科を受診した。左眼視力は光覚弁±で、前眼部には異常がなかった。左眼に網膜の混濁，網膜中心動静脈の血流途糸色網膜前と網膜下の出血，乳頭前の硝子体出血があった。受傷8日後の螢光眼底造影で，脈絡膜には血流があったが，網膜血管には血流が途絶していた。鈍性外傷で網膜中心動静脈の閉塞が起こりうること，そして乳幼児では一見軽微な外傷でも，注意が必要であることを示す例である。

A 2-year-old girl was hit by the corner of desk top. She showed slight laceration of the eyelids and susbcutaneous hemorrhage. When seen two days later, the anterior ocular segment was normal. Visual acuity was light perception. The left eye showed opaque retina, pre- and subretinal hemorrhage, absence of circulation in central retinal artery and vein, and vitreous hemorrhage. Fluorescein angiography 8 days after injury confirmed the diangosis of occlusion of central retinal artery and vein. This case illustrated that occlusion of central retinal artery and vein may occur after blunt trauma without involving the external eye segment.
