ゴルフボール打撲による眼球破裂の1例

A case of scleral rupture by a flying golf ball

　硝子体出血及び網膜剥離に対し硝子体手術を行った際，眼球赤道部より後方に位置する強膜裂創が発見されたゴルフボールによる眼外傷の1例を報告する。強膜裂創の処置と硝子体手術を同時に施行し，網膜を復位させ，術後0.2の矯正視力が得られた。本症例の様に受傷直後に眼球破裂の診断が困難な症例に対する注意点について検討を行った。

42-year-old male was hit by a flying golf ball in his left eye. Surgical exploration failed to identify rupture of the eyeglobe. Vitreous hemorrhage per-sisted and retinal detachment was detected by echography 40 days after injury necessitating a second surgery. We identified scleral rupture 9.5 mm in length 16 mm posterior to the limbus. Addi-tional vitrectomy led to reattachment of the retina and recovery of visual acuity to 0.2. Retinal fold due to traction by proliferation from scleral rupture precluded further improvement in vision.
