A 47-year-old female presented with blurring in the right eye of 20 day's duration. She had the past history of recurrent horizontal hemianopia in the right eye, paraplegia due to peripheral neuritis and cerebral infarction. Biopsy of sural nerve had shown vasculopathy with organized thrombosis surrounded by infiltration of phagocytes, leading to the diagnosis of primary antiphospholipid antibody syndrome (PAPS) . Funduscopy showed retinal edema, hard exudates and flame-shaped hemorrhages in the right eye. Fluorescein angiography showed leakage and dye staining along retinal arteries. The affected eye was diagnosed as retinal vasculitis presumably secondary to thrombus formation as a manifestation of PAPS.
