Triple Procedureにより視力改善を得たDown症候群に球状角膜を合併した1例

Improvement of visual acuity after triple procedure in a case of Down's syndrome with keratoglobus

　球状角膜と白内障を合併した36歳のDown症候群の1例を報告した。両角膜は半球状に高度突出，菲薄化しており，右眼には角膜中央部の実質混濁と成熟白内障を認めた。右眼に対しtriple procedureを施行した結果，行動様式が著しく改善した。切除角膜の組織切片では一部にBowman膜の断列像と直下の実質に膠原線維の配列の乱れが認められた。

　Down症候群は眼合併症の頻度が高いにもかかわらず，眼科受診の機会は少ない。Down症候群の高齢化が進む現在，今後このような症例に対しても積極的な治療を行っていくことが必要であると考えられた。

A 36-year-old female with Down's syndrome presented with typical features of keratoglobus in both eyes. She also manifested mature cataract and central corneal stromal opacity in her right eye. We performed triple procedure of penetrating kerato-plasty, cataract extraction and intraocular lens implantation in the right eye. Her daily behavior showed remarkable improvement. The resected cornea showed occasional breaks in the Bowman's membrane and disarrangement of stromal collagen fibrils. As the lifespan of individuals with Down's syndrome is on the increase, due therapeutic atten-tion is necessary for such cases.
