A 36-year-old female with Down's syndrome presented with typical features of keratoglobus in both eyes. She also manifested mature cataract and central corneal stromal opacity in her right eye. We performed triple procedure of penetrating kerato-plasty, cataract extraction and intraocular lens implantation in the right eye. Her daily behavior showed remarkable improvement. The resected cornea showed occasional breaks in the Bowman's membrane and disarrangement of stromal collagen fibrils. As the lifespan of individuals with Down's syndrome is on the increase, due therapeutic atten-tion is necessary for such cases.
