両眼性サルコイド網膜肉芽腫の1症例

A case of bilateral sarcoid retinal granulomas

　サルコイド網膜肉芽腫の発生過程を，検眼鏡と螢光眼底撮影にて詳細に観察し得た18歳女性の1症例を報告した。肉芽腫は両眼性で，滲出性網膜剥離を伴った黄白色の隆起性病変として6か所に出現した。螢光所見上，早期に低螢光，後期に旺盛な色素漏出を示し，また，ステロイドの全身投与が著効した。これらの所見は，今までのサルコイド脈絡膜肉芽腫の報告と一致していた。しかし，本症例にみられたすべての肉芽腫は，初診時に網膜静脈周囲炎のみられた部位に一致して発生し，消退後には，網膜色素上皮萎縮などの変化を残さなかったことより，網膜起源であることが強く示唆された。したがって，今回の検討から，過去に脈絡膜肉芽腫と診断されたものの中には，網膜肉芽腫が含まれている可能性があると思われた。

A 18-year-old female presented with bilateralretinal granulomas which proved to be manifesta-tions of ocular sarcoidosis. The granulomas appear-ed as elevated yellowish white retinal masses withexudative retinal detachment. In fluorescein angio-graphy, the lesions blocked the background fluores-cence in the arteriovenous phase and showedintense staining in the late venous phase. Thegranulomas disappeared after systemic corticoster-oid therapy.

Above fundus features simulated those of so-called choroidal granulomas. In the present case,retinal granulomas developed in regions whereretinal periphlebitis were located earlier. Theretinal pigment epithelium showed minimum alter-ation in areas where granulomas were locatedearlier. These findings seemed to suggest that someof presumed choroidal sarcoid granuloma mayhave been actually instances of sarcoid retinalgranulomas.
