Out of a consecutive series of 312 eyes that underwent penetrating keratoplasty during a 13 -year period through 1987, there were 20 patients in whom keratoplasty was performed later on the fellow eye. We evaluated the outcome in these 20 bilateral cases.

Postsurgical clear grafts accounted for 62.5% of the eyes. There was no statistically significance between the incidence of clear graft in the first eye(55%) and the second eye (70%).

When the second eye was operated in less than one year after surgery in the first eye, the graft remained clear in 44% of the eyes. When surgery was performed later than one year, the graft remained clear in 77.3% of the eyes. The difference was significant.

The incidence of clear graft was not dependent on the age of the donor, either for the first or the second operation.
