超音波乳化吸引術による術前術後角膜倒乱視例の解析．その1

Induction of postcataract astigmatism after phacoemulsification procedure

　白内障の手術成績を単純化した条件で検討する目的で，同一時期，同一術者，同一方法でなされた白内障手術を検討した。

　術前倒乱視を示し，かつ6か月間観察された症例は313眼，一方，術後倒乱視を示し，6か月前にさかのぼって各測定時期のすべての記録のあった症例は257眼であった。全総数は1,077眼で，KPE 809眼（76.3％），ECCE 217眼（20.5％），その他34眼（3.2％）である。

　KPE法に限定し，術前および術後の角膜倒乱視を解析した。術前倒乱視群では1週後の直乱視は67.3％であり，6か月後では7.9％であった。70歳以下と80歳以上を比較すると，年齢が低いほど，術後1週目での強主径線の収縮が強く，また早く改善した。年齢による術後の乱視度に差はなかった。以上の結果は以前に報告されているECCEの術後変化と必ずしも一致しなかった。

Induction and regression of corneal curvature in the first six months after cataract extraction were studied by performing keratometry. Pre-operative astigmatism was found in 313 cases, and 257 pos-toperative AR astigmatism was also examined.

Surgery was done by one surgeon with the same surgical procedure using 10-0 mersilene (polyes-ter). An early and pronounced shift in the axis of astigmatism to direct astigmatism was more remarkable in younger patients (≦70 years old) than in older ones (≧80 years old). The recovery of corneal curvature was also faster in the younger patients.

These characteristies induced by KPE procedure was somewhat different from the ECCE procedure already reported.
