網膜色素変性症における白内障手術と眼内レンズ挿入

Cataract extraction and intraocular lens implantation in patients with pigmentary retinal dystrophy

　定型的網膜色素変性症8名の10眼に白内障手術と眼内レンズ挿入術を行った。4眼には紫外線吸収眼内レンズを挿入し，6眼には一般の眼内レンズを挿入した。術後10眼中9眼に，0.3以上で，かつ2段階以上の視力改善が得られた。1眼では黄斑の変性のため視力は改善しなかった。1眼で術後4週間に類嚢胞黄斑浮腫がみられたが，術後約12か月で2段階の視力改善が得られた。術後には全例に遮光矯正眼鏡の装用をすすめた。術後に視力が改善しなかった例も含めて，全例で明所における昼盲の症状が軽減した。これまでの観察で，紫外線吸収レンズ挿入群と通常のレンズ挿入群との間に，術後の経過の差はなかった。

Cataract surgery and intraocular lens implanta-tion in 10 eyes of 8 patients with pigmentary retinal dystrophy was carried out. Four eyes were implant-ed with ultraviolet-absorbing intraocular lens and 6 eyes were implanted with ordinary intraocular lens. Nine eyes showed visual acuity of 0.3 or better postoperatively. One eye showed cystoid macular edema 4 weeks after surgery.

Patients were recommended to wear corrective coloured glasses after cataract surgery. With glas-ses, all patients claimed less daylight-blindness in the daylight when compared to before surgery. No clinical difference was observed between eyes with ultraviolet-absorbing intraocular lens and eyes with ordinary intraocular lens.
