眼球鉄症における水晶体および虹彩の病理組織学的研究

Histopathology of the lens and iris in a case of ocular siderosis

　眼球内鉄片異物により眼球鉄症が生じた症例に水晶体嚢内摘出術および周辺虹彩切除術を施行し，水晶体および虹彩根部を病理組織学的に観察した。ヘマトキシリン—エオジン染色（H．E．染色）による標本では，前嚢下の水晶体上皮細胞が紡錘形の形態をとり増殖していた。前嚢下および後嚢下の皮質線維は変性し，線維構造を失って均質無構造な物質となっていた。また水晶体上皮細胞内および上皮細胞下の皮質に褐色の色素が沈着していた。

　虹彩は前色素上皮細胞層の筋突起である瞳孔散大筋に褐色の色素が沈着していた。

　これらの褐色の色素沈着に一致してベルリン青染色では青く染まり，鉄の沈着が確認された。

We performed intracapsular cataract extraction and iridectomy in a 33-year-old male with siderosis bulbi. The intraocular ferrous material had been retained for 2 years before its removal one yearbefore. The surgically obtained lens and the iris were subjected to histopathological studies. Stain-ing with hematoxylin and eosin showed brown pigment in the lens epithelium, lens cortex and dilator muscle of the iris. These lesions were also positive for Berlin blue stain.
