眼症状で発症し脳血管内手術が著効した脳血管障害の2例

Endovascular surgery improved ocular findings in two cases of cerebral vascular lesions

　27歳女性が頭痛と複視，26歳女性が複視と右眼球突出を主訴として受診した。精査により，それぞれ硬膜動静脈奇形，外傷性内頸動脈海綿静脈洞瘻と診断された。両症例ともマイクロコイルによる脳血管内手術を受け，翌日から眼症状がほぼ消失した。複視など多彩な眼症状があるときには，脳血管障害がその原因で，侵襲の小さい血管内手術が有効でありうることを示す症例である。

A 27-year-old female presented with headache and diplopia. She was diagnosed with dural arteriovenous malformation. Another 26-year-old female presented with diplopia and right proptosis. She was diagnosed with trau-matic carotid-cavernous sinus fistula. Following endovascular surgery using microcoils, the ocular manifestations dis-appeared the next day in both patients. These cases illustrate that intracranial vascular lesions may cause acute ocular disorders including diplopia and that noninvasive endovascular surgery may be the treatment of choice.
