ヌンチャク型シリコーンチューブ挿入例361例の検討

Review of 361 cases inserted with nunchaku-style silicone tube

　涙道再建術としてヌンチャク型シリコーンチューブの留置術を361例に行った。鼻涙管閉塞252例，慢性涙嚢炎41例，涙小管閉塞36例，鼻涙管狭窄18例，その他である。再挿入53例を含め，鼻涙管閉塞と涙小管閉塞では80％，鼻涙管狭窄では全例が改善した。慢性涙嚢炎と涙嚢鼻腔吻合術後の吻合部閉塞の諭54例では，初回挿入による改善が30％で得られ，再挿入で約半数が改善した。シリコーンチューブの留置期間が長いほど改善率が高い傾向が全疾患であり，鼻涙管閉塞ではこれが有意であった。重篤な合併症はなかった。ヌンチャク型シリコーンチューブの留置術は安全で有効な涙道再建術であると結論される。

A total of 361 cases received insertion of nunchaku-style silicone tube. The series comprised nasolacrimal duct obstruction 252 cases, chronic dacryocystitis 41, obstruction of lacrimal canaliculi 36 and stenosis of nasolacrimal duct 18 among others. The tube had to be re-inserted in 53 cases. The outcome was a success in all the cases of naso-lacrimal stenosis, and in 80% of cases of obstruction of nasolacrimal duct or lacrimal canaliculi. In cases of chronic dacryocystitis and obstructed lacrimal passage following anastomosis surgery, success was obtained in 30% after ini-tial which improved to 50% after re-insertion. There was a tendency for better outcome when the tube was retained longer. This tendency was significant in nasolacrimal duct obstruction (p＜0.05). There has been no complication of note throughout. It is concluded that the present method is safe and effective in the reconstruction of lacrimal passage.
