慢性涙囊炎に対するブリムヌンチャク型シリコーンチューブの挿入

Brim-type nunchaku-style silicone tube in the treatment of chronic dacryocystitis

目的：鼻涙管閉塞に対するブリムヌンチャク型シリコーンチューブ挿入術の報告。対象と方法：慢性涙囊炎がある鼻涙管閉塞10例に，従来の改良型である単管式ブリムヌンチャク型シリコーンチューブを挿入した。全例が片側性であり，年齢は62～82歳（平均72歳）であった。3～8か月間チューブを留置し，抜去後平均3か月間の経過を追跡した。結果：抜去後の再閉塞は皆無であり，全例で排膿が改善していた。結論：慢性涙囊炎がある鼻涙管閉塞に単管式ブリムヌンチャク型シリコーンチューブ挿入は有効であった。チューブそのものによる狭窄や閉塞が軽いことがその理由と考えられる。

Purpose：To report the outcome of treatment of obstructed nasolacrimal duct by brim-type nunchaku-style silicone tube. Cases and Method：Ten patients of obstructed nasolacrimal duct with chronic dacryocystitis received insertion of brim-type nunchaku-style silicone tube. All the patients were unilaterally affected. Their ages ranged from 62 to 82 years,average 72 years. The tube was retained for 3 to 8 months. Cases were followed up for an average of 3 months after removal of the silicone tube. Results：There was no case of reobstruction after removal of the tube. Discharge of pus improved in all the cases. Conclusion：Insertion of brim-type nunchaku-style silicone tube was effective for obstructed nasolacrimal duct with chronic dacryocystitis. The beneficial effect may be due to lesser obstruction or occlusion by the tube itself than by previous one.
