We evaluated the incidence of reopening of macular hole after vitreous surgery in 283 eyes. The internal limiting membrane (ILM) was left intact in 160 eyes operated during 4 years through 1996. The ILM was removed in 123 eyes operated during 3 years through 2001. In eyes with intact ILM, the macular hole closed in 69% after initial surgery and finally in 91%. In eyes with removal of ILM, it closed in 95% after initial surgery and finally in 100%. The macular hole reopened in 5% of eyes with intact ILM and in 0% of eyes with removed ILM. The cause of reopening was mainly due to formation of epiretinal membrane. It appears that removal of perifoveal ILM would promote closure of macular hole by releasing the tangential traction and would also prevent reopening by suppressing formation of epiretinal membrane.
