正常眼圧緑内障が疑われたempty sella症候群の2症例

Two cases of empty sella syndrome masquerading as normal-tention glaucoma

　緑内障経過観察中にempty sellaを認めた2症例を経験した。2症例ともに，MRI検査上，極めて類似したempty sellaの所見を認めたが，症例1は両鼻側半盲様の視野変化から頭蓋内病変が主体と考えられ，症例2は視神経・視野変化の様式から緑内障性変化が主体と考えられた。正常眼圧緑内障（NTG）の鑑別疾患の1つとしてempty sella症候群が挙げられるが，NTGを疑った症例にempty sellaを認めた場合，診断や治療方針の決定に際して視神経と視野の所見から総合的に判断するべきで，NTGを一律に除外すべきではないと考えられた。

We diagnosed two cases as empty sella syndrome based on magnetic resonance imaging (MRI).Both had been inititally diagnosed as normal-tension glaucoma. One case manifested binasal hemianopia which appeared to be secondary to intracranial lesions. The other showed typical glaucomatous changes in the visual field and optic disc in both eyes. These cases illustrate that empty sella syndrome and normal-tension glaucoma may occasionally co-exist and that this possibility would merit due consideration in the diagnosis and therapeutic approach.
