Abstract. A 61 year-old male presented with impaired vision in his right eye since one week before. His corrected visual acuity was 0.6 right. His right eye showed cherry-red spot and soft exudates around the optic disc. Fluorescein angiography showed delayed dye filling in the retina and choroid. The left eye showed normal funduscopic and angiographic findings. Magnetic resonance angiography showed patient internal carotid and ophthalmic artery. We started daily intravenous infusion of urokinase under the assumption of impaired circulation in the distal portion of ophthalmic artery. Ten days later,the fundus findings improved with the visual acuity of 1.2. Fluorescein angiography showed improved retinal and choroidal circulations.This case illustrates that systemic urokinase may be effective for presumed impairment in the ophthalmic artery with minimally affected visual acuity.
