在宅医療における眼科訪問診療

Home care of eye patients by visiting ophthalmologic personnel

　在宅医療の一環として当院で取り組んでいる眼科訪問診療について検討した。1998年4月から2年間で，在宅医療対象者38名のうち眼科訪問診療を行ったのは22名で，平均年齢は83歳であった。診療動機の大部分は白内障による視力障害であった。4名7眼に白内障手術を行った。眼科訪問診療を行い他科と連携することで，生活の質を考慮した全人的医療が可能となり，通院の困難な患者に対する視機能改善の機会が増えた。問題点として，十分な検査ができにくい，診療時間の確保の問題などがあった。超高齢社会の到来とともに在宅医療における眼科のニーズは高まると予想され，プライマリケアを担う医療機関の眼科においても，今後対応の拡充が必要と思われる。

We reviewed out program of home care of eye patients during the past 2 years. Out of 38 patients staying athome, 22 were cared for by visiting ophthalmologic professionals. Their ages ranged from 61 to 95 years, average 83 years. The majority had cataract as the cause of visual impairment. Eventual cataract surgery was performed in 7 eyes of 4patients. In addition to other treatments for systemic diseases, home care by visiting ophthalmologic personnel improvedthe quality of life of patients. Although our home care involves problems as examinations have to be performed within alimited time, ophthalmological staff in primary care hospitals should more participate in providing home care service infuture.
