Purpose: To review the outcome of Molteno implant surgery for refractive glaucoma. Cases: Surgery was performed on 15 eyes of 14 patients with refractive glaucoma during the foregoing 8 years. The series comprised diabetic retinopathy with neovascular glaucoma 5 eyes, retinal vein occlusion 3 eyes, and one case each of familial exudative vitreoretinopathy, uveitis, lens particle glaucoma, iridocorneal endothelial syndrome, uveitis, primary open-angle glaucoma, and combined juvenile and steroid glaucoma. Eleven eyes had received trabeculectomy before.

Results: After follow-up averaging 505 days, 12 eyes (80%) showed control of intraocular pressure (I0P) between 6mmHg and 21mmHg. MP remained over 21mmHg in one eye (6.7%) . Phthisis bulbi resulted in 2 eyes (13%) . Major complications included flat anterior chamber 6 eyes (40%) , choroidal detachment 5 eyes (33%), and attachment of the tube to the corneal endothelium 3 eyes (20%). Conclusion : Molteno implant surgery may induce a high rate of IOP control in eyes with refractive glaucoma even after conventional surgery has proved futile.
