網膜振盪症の受傷後の視機能を評価する目的で,金沢大学医学部附属病院眼科を受診し,9か月以上経過を観察した2例2眼を対象として,走査レーザー検眼鏡(SLO)マイクロペリメトリーを行った。受傷直後に白色網膜混濁を呈する部位,すなわち広義の網膜振盪症が認められる部位に絶対暗点,相対暗点および正常感度の領域が混在していた。受傷直後に絶対暗点を呈した部位は,後に網脈絡膜萎縮を示して絶対暗点のままであり,受傷直後に相対暗点を呈した部位は,脈絡膜破裂を起こした部位を除いて正常の感度に回復した。
We examined 2 eyes of 2 adults who developed commotio retinae after being hit while playing baseball. Both eyes showed Berlin's edema and contusion necrosis immediately after the accident. These eyes were examined by microperimetry using a scanning laser ophthalmoscope (SLO) . The initially turbid retinal areas showed various degrees of sensitivity loss ranging from absolute scotoma to normal sensitivity arranged in a mosaic pattern. The absolute scotomatous areas later turned into chorioretinal atrophy which was devoid of light sensitivity. Relative scotomatous areas excepting choroidal rupture recovered normal sensitivity during the ensuing 9 months. The findings show the clinical value of SLO microperimetry in predicting the final functional outcome in commotio retinae.
